Program description
Patients with Type 2 diabetes, who have a smart phone and internet access, sign up for the program through a website. They detail their condition - medications, activity level, dietary challenges, etc. Based on this questionnaire, they will receive daily texts - prompting them to exercise, take their insulin, making a healthy choice. Most patients had very positive feedback, many improved their diabetes control.
